Population Health & Value-Based Care SME
Why You'll Actually Care
District Partners is exclusively supporting a rapidly growing, investor-backed healthcare SaaS company at an inflection point where deep population health expertise needs to sit much closer to the technology itself.
The platform helps provider organizations turn complex clinical, financial, and operational data into better quality outcomes, stronger care coordination, and improved performance under value-based reimbursement models.
What makes this role different is the need for someone who brings current, hands-on depth in population health and MIPS, not simply broad healthcare leadership or experience managing teams that owned the work. You'll become a critical voice connecting what providers are dealing with every day to the products, workflows, and decisions that can solve those challenges at scale.
The Basics
This is a highly visible, strategic individual contributor role sitting at the intersection of population health, MIPS and quality performance, value-based care, healthcare data, and technology.
You'll work directly with executive leadership, Product, Engineering, Implementation, and Customer Success, bringing real-world healthcare expertise into product strategy, customer conversations, and execution.
What You'll Be Owning
Become a key internal authority on population health, MIPS, healthcare quality, and value-based care, bringing practical subject matter expertise into product and customer decisions
Translate complex population health workflows, MIPS requirements, quality measures, and healthcare data into scalable technology capabilities and actionable product direction
Influence product strategy by connecting real-world provider challenges, regulatory requirements, reimbursement models, quality initiatives, and data requirements to future platform enhancements
Work directly with executive leadership and customers on complex population health, quality performance, care management, and value-based care challenges
Support Implementation and Customer Success with the subject matter depth needed to improve adoption, quality performance, and long-term customer outcomes
Stay actively involved in the substance of the work, from customer discussions and workflow challenges to product roadmap decisions and emerging healthcare requirements
The Environment
This is not a traditional population health operations role, and it is not a people leadership position where the subject matter sits several layers below you.
The company needs someone who has remained close to population health and quality work and can personally get into the details of MIPS, HEDIS, CMS/Stars, NCQA, provider workflows, quality measures, healthcare data, and value-based reimbursement.
You'll have significant executive exposure, but credibility will come from combining that presence with current domain expertise and the willingness to stay hands-on.
What You Bring To The Table
Deep, current experience in population health and value-based care, with a strong understanding of provider operations, care management, quality performance, and reimbursement
Hands-on MIPS expertise with the ability to personally speak to requirements, measures, reporting, workflows, challenges, and their implications for provider organizations
Strong knowledge of HEDIS, CMS/Stars, NCQA, and related healthcare quality measurement and reporting frameworks
Strong understanding of healthcare data and analytics, including the clinical, quality, operational, and claims data used to measure population health and value-based performance
Experience working with health systems, physician groups, ACOs, CINs, health plans, or other organizations managing population health and value-based performance
Experience with enterprise healthcare technology, population health platforms, EHRs, care management solutions, analytics platforms, or related healthcare applications
Ability to translate complex healthcare workflows, data, quality requirements, and provider needs into practical direction for Product and Engineering teams
A career trajectory that may include leadership responsibility but has not taken you away from the actual subject matter and work
Executive-level communication skills with the ability to translate complex healthcare concepts for clinical, operational, product, technical, and senior leadership audiences
